This form is provided as a convenient way fo you (the user) to tell us (the systems office) about computer problems you encounter in the libraries.
All information is "required" if corrective action is to be taken. Please remember that the more specific you are about the problem, the more likely we are to duplicate, indentify, and correct the problem.
